【EXPLANATION OF IMPORTANT INFORMATION】CareEarth Mobile
In accordance with Article 26 of the Telecommunications Business Act (Explanation of Service Conditions), we hereby provide the following information regarding our services. These details are important, so please read them carefully before proceeding with your registration.

PURPOSE OF PERSONAL INFORMATION USAGE
1. Based on Article 54 of the CareEarth Mobile Terms of Service (hereinafter referred to as the "Terms of Service"), we use personal information for purposes such as identity verification, contract conclusion/execution/termination, changes to fees/service conditions, delivery, suspension of service, cancellation/termination notifications, billing, asset/equipment setup and maintenance, related after-sales services, improvement and development of products/services, and providing information about products/services via email, direct mail, telephone, in-person visits, and other methods. We may also use personal information as necessary for conducting and implementing surveys and related tasks.
2. To the extent necessary to fulfill the purposes outlined in item 1, we may disclose or provide personal information to subcontracted companies and other telecommunications service providers.
ABOUT THE SERVICE CONTRACT
● Before applying to use the CareEarth Mobile service (hereinafter referred to as the "Service"), please carefully read, confirm, and agree to the Terms of Service posted on the official website.
● If there are errors in the contract details (e.g., name, address, contact information), the contract may be cancelled.
● To prevent registration using false identity verification information, we will check personal identification documents such as residence cards or driver’s licenses at the time of SIM card application. We may take necessary actions, such as submitting identification information to issuing authorities or requesting reference checks.
SERVICE INFORMATION
● This service uses the network infrastructure of NTT Docomo, Inc.
● This (D) plan is a mobile internet and voice call service that uses the "Xi®" and "FOMA®" networks provided by NTT Docomo, Inc. in Japan.
● This is a best-effort service. Even if a specific transmission speed is indicated, it does not guarantee the maximum achievable speed. The actual speed may fluctuate or degrade depending on factors such as the network system, radio wave conditions, SIM card, connection status, and device capabilities. Please check the technical specifications of your device for its signal reception capabilities.
● For coverage areas, please refer to the following page: https://www.nttdocomo.co.jp/corporate/disclosure/mvno/user
● Even within service coverage areas, the service may be unavailable in locations with poor signal reception such as inside high-rise buildings, underground, in tunnels, or in outdoor areas with weak signals.
● Services exclusively provided by NTT Docomo, Inc. (such as i-mode®) are not available.
● Devices used with this service must meet the following conditions:
● Be compatible with the 5G, "Xi®", or "FOMA®" networks provided by NTT Docomo, Inc.
● Have a Technical Conformity Mark (indicating compliance with designated wireless equipment technical standards).
● If you are using a SIM-locked device not issued by NTT Docomo, it must be SIM-unlocked.
● You will need to configure the APN (Access Point Name) settings on your device or install the APN configuration profile.
● Please note that SIM cards included in a purchased service package are non-refundable and cannot be returned.
● Please also note that cancellations of service applications are not accepted.
● The SIM card is provided as a loaned item.
● If you are using an eSIM, you will need to set up the eSIM profile.
● Only individuals aged 18 and older are eligible to apply for and use this service.

<Regarding Usage Restrictions>
● In the event of a natural disaster, accident, or other emergencies — or if such events are likely to occur — we may restrict transmission speed or duration of use in certain areas due to network congestion.
● If the duration or volume of data transmission exceeds a specific threshold, data transmission may be restricted or disconnected.
● If you send or receive large files (such as attachments or downloaded videos), or if you engage in continuous data transmissions such as streaming or video content, your transmission speed may be temporarily reduced.
● To maintain overall network quality and manage bandwidth for specific categories of applications, measures such as image compression and video download speed adjustment may be applied. Please note that compressed images cannot be restored. These restrictions are not subject to removal.
● File-sharing applications (P2P) are not permitted.
● This service will be assigned a private IP address.
● Some applications may not function properly due to the use of a private IP address.
● Depending on the customer’s device, unintended signal issues may occur.
● Provisions related to usage restrictions may be changed after registration.
● Other usage restrictions may apply in accordance with the terms and conditions of the mobile network provider.
<Service Plans>
● Customers can use both wireless data transmission and voice call services. However, for SMS functionality, it is only available if the selected plan includes SMS service. Voice calling services can only be used with plans that specifically include calling. Please refer to the attached "CareEarth Mobile Pricing Table" for more details.
● Each plan has a designated amount of available data or a defined period of high-speed data usage. Please refer to the attached information sheet for further details.
● Please note that data usage is calculated based on monthly transmission volume. Usage from the end of the current month to the beginning of the next month may be counted toward the following month’s total.
● If you wish to change your plan, a separate application must be submitted to our company. Plan changes will only take effect upon our approval and must be in accordance with the specifications outlined in the attached table. However, you cannot change the type of service (e.g., from data-only to voice + data or vice versa)
● Plan changes can be requested from the month following the initial service start date. You cannot request a plan change within the same month your service begins.
● After a plan change is accepted, the new plan will be applied from the month following the change request.
● Please be aware that any discounts associated with your previous plan may end upon switching to a new plan.
● For the "Unlimited Call Plan" option, if you are transferring your number via MNP (Mobile Number Portability), the unlimited call feature will be available from the day after your service begins. Calls made on the service start date will be charged according to your contract with the previous provider.
<About Voice Call / SMS Services>
● When applying for a voice call plan, we are required by the Act on Prevention of Improper Use of Mobile Phones to verify your identity. Therefore, please provide valid identification documents at the time of application. Please note that identification documents will be retained for three years after service cancellation.
● Once your identity verification is completed, the SIM card will be delivered to the address stated on your identification documents. If the identity verification cannot be completed within 30 days from the date of application, your application will be cancelled.
● If you are applying via the Mobile Number Portability (MNP) system, please refer to the section <For Applications Involving MNP> below.
● Customers subscribing to a voice call or SMS plan may use SMS (Short Message Service), but MMS (Multimedia Messaging Service) is not supported.
● Please note that if your contract is terminated due to unpaid fees, your current phone number used with this service will be cancelled.
● The network PIN is printed on the SIM card package.
● A MNP reservation number for transferring from this service (if a new number is issued) may be available starting from the 8th day after your service begins.
《About CareEarth Mobile Voice Call Services》
● Customers who select a voice call plan with this service may use CareEarth Mobile's voice call service. There are two types of CareEarth Mobile voice services: the "CareEarth Mobile Voice Call Plan" and the "Flat-rate Call Option." The "CareEarth Mobile Voice Call Plan" is available without a monthly basic fee (call charges are not included).
● Customers who wish to use the flat-rate call option must apply separately and may use the service upon approval by our company.
● If you place calls sing the ENPORT Mobile Voice Call service, your usage will be subject to the terms and conditions established by both our company and the service provider. Additionally, charges for calls made using the ENPORT Mobile service will be applied in accordance with the CareEarth Mobile service terms.
● The CareEarth Mobile voice call service is provided as a bundle with the overall voice calling service and cannot be cancelled separately.
● When using CareEarth Mobile's voice call service to make international calls, the monthly usage limit is set at ¥20,000.
● For domestic calls made under a voice plan, all calls will be routed through CareEarth Mobile's voice service by default, unless you voluntarily add a prefix number or the destination number is unreachable via our service.
《About International Roaming》
● Customers using a voice plan with our service may access international roaming by submitting a separate application and receiving approval from our company.
● International roaming services may be restricted due to foreign laws or regulations or by the terms and conditions of overseas telecom carriers.
● The monthly usage limit for international roaming is set at ¥50,000. If this limit is exceeded, your international roaming service will be suspended from the time the overage is confirmed until the end of the month.
● If it is confirmed that the usage limit has been exceeded within a 24-hour period, we may suspend international roaming until a request for resumption is received from the
customer.
《About International Calling》
● Customers who have subscribed to a voice plan and been granted access to voice calling may use international calling services upon separate application and approval by our company.
● The monthly usage limit for international calls is set at ¥20,000. Once this limit is exceeded, the service will be suspended until the end of the current billing month. We may impose or lower usage limits depending on your payment status for international call fees and any other outstanding balances.
● If the customer falls under any of the following conditions, we may suspend international calling services for a period determined by our company.
<Suspension of International Call Services>
We may suspend your international call service for a period determined by our company if any of the following situations apply:
● When we are unable to confirm payment of international call service charges or other outstanding debts even after the payment due date has passed. This includes cases where payment was not made using the method designated by our company, or where payment was made after the due date but confirmation of receipt could not be verified.
● When false or inaccurate information is found in the registration details for the international call service.
● When there is a change from the initially provided information without notification to our company, or when we discover that the reported information was false or misleading. When the user engages in acts that obstruct, or are likely to obstruct, our telecommunications equipment related to our business or international call services.
● When the use of international call services causes significant disruption to other customers.
● When the international call services are used fraudulently or without proper authorization.
● In addition to the above, when the user violates any provisions stipulated in the Terms of Use.
<For Applications Involving Mobile Number Portability (MNP)>
● Please ensure you apply at least 10 days before the expiration of your MNP reservation number (yoyaku bangō). If less than 10 days remain before the expiration, MNP registration will not be possible.
● If identity verification cannot be completed within the final 10 days before the expiration of your MNP reservation number, your application will be cancelled.
● Please note that if you are transferring from another carrier using MNP, you will be unable to use this service or your previous services during the period between registration and SIM card delivery.
● Aside from the phone number, other data (e.g., mobile email addresses issued by your former carrier) cannot be carried over.
<About Optional Services>
As optional features of this service, customers may purchase additional data (e.g., 1GB increments). For pricing details of each optional service, please refer to the attached “CareEarth Mobile Pricing Table”
《About Additional Data Purchases》
● Customers may purchase additional data when the monthly data limit set by the selected plan is exceeded. Once the maximum amount of purchased data is reached, data transmission will proceed at a separately defined reduced speed.
● The validity period for purchased additional data is until the last day of the third month following the month of purchase. After this period, any unused additional data will expire and become unavailable.
● If multiple additional data purchases have been made, the data with the nearest expiration date will be consumed first.
● If the service is cancelled while additional data remains, the remaining data balance will not be refunded.
《About Data Rollover》
● If you are subscribed to a plan that allows data rollover and your data usage within a given month is below the plan’s data allowance, the remaining unused data will be carried over to the following month. This rolled-over data will be valid until the last day of the next month.
<Order of Data Usage>
Available data will be consumed in the following order:
1. Rolled-over data
2. Monthly plan data allowance
3. Paid additional data
《About Flat-rate Calling Service》
● After starting to use this service, if you apply for the Hōdai (flat-rate calling) option, you may register between the 1st and the 20th of each month, and the option will become active starting from the following month. Please note that calls made during the month of registration will be charged based on
standard call rates. Registration must be completed via My Page.
<About Service Fees and Other Charges>
● This service includes a basic monthly usage fee, a universal service fee, and a telephone relay service fee. No additional charges will be applied beyond the basic usage fee. However, this policy is subject to change. In such cases, we will notify customers in advance and appreciate your understanding.
● If optional services are used, charges for those optional services will apply.
● No contract termination fees will be imposed. However, this policy is also subject to change, and any changes will be announced in advance.
● If payment is not confirmed by the due date each month, a late payment fee of 330 yen (tax included) will be added to the amount due when the payment request is reissued.
● If the SIM card is not returned or is damaged, a replacement fee may apply. For details regarding fees, please refer to the attached "CareEarth Mobile Price List."
● Call charges and international roaming fees are billed separately for voice calls.
● If you change your plan, the fee for the new plan will be charged starting from the month in which the new plan begins.
<About Suspension and Resumption of Service>
● To suspend or resume usage of your SIM card, please contact the ENPORT Mobile Support Center.
● Please allow up to two business days for suspension or resumption procedures to be completed after submitting your request.
INITIAL COSTS, USAGE FEES, AND PAYMENT METHODS
● No initial costs will be charged for subscribing to CareEarth Mobile services.
● Please check the monthly usage fee on the My Page provided by our company. If you use convenience store payment, you can also confirm the fee in the monthly billing email.
● You can view contract and usage details on the My Page provided by our company.
● If you require printed documents such as an invoice, payment certificate, or any official written statement, a fee of 550 yen (tax included) will apply.
● If SIM reissuance is required due to reasons attributable to the customer, the fee will be 2,200 yen (tax included).
● For new subscribers to CareEarth Mobile, charges will begin from the month in which the customer has designated to receive the SIM card. The usage fee for the first month will be calculated on a daily basis, using a 30-day month as the standard reference.
《Credit Card》
● Usage fees will be charged by your credit card company on the date they designate.
《Convenience Store Payment》
● A payment request email will be sent to your registered email address on the 8th of each month.
● Please log in via the URL provided in the email, select Payment → Convenience Store Payment, and input the displayed payment code into a Loppi or Multi-Copy machine at the convenience store.
● Please make your payment at the cashier and keep the receipt.
《At the Time of SIM Registration》
● Payment Request: Once identity verification is complete and there are no issues, a payment request email will be sent.
● Payment Deadline: Payment must be made within 7 days from the date the email is sent.
Note: If payment is not confirmed within 7 days, your data will be deleted and re-registration will be required if you wish to reapply.
《Monthly Payments》
● Email Notification: A payment request email will be sent to your registered email address on the 8th of each month.
● Procedure: Log in to My Page from the email link, select Payment → Convenience Store Payment, and enter the displayed payment code into a Loppi or Multi-Copy machine.
● Deadline: Payment must be completed by the 22nd of the same month.
Late Payment Handling
Late Payment (Stage 1)
● If payment is not confirmed by the 22nd, a second payment request will be issued on the 23rd.
● A late fee of 330 yen (tax included) will be added.
● Extended Payment Deadline: Until the end of that month.
Late Payment (Stage 2)
● If payment is still not confirmed by the end of the month, the unpaid amount will be rolled over and combined with the next month’s usage fee.
● A new payment request for the combined amount will be sent on the 8th of the following month.
● Deadline: 22nd of that following month.
● If payment is still not confirmed, the contract will be forcibly terminated due to non-payment for two consecutive months, and the customer’s record will be registered on a delinquent user list (network blacklist).
Important Note: If your contract is forcibly terminated, you will not be able to retain your current phone number.
Reference
● Regular Monthly Payment: Payment request sent on the 8th each month, payment due by the 22nd.
● Late Payment Stage 1: Monthly fee + 330 yen late fee billed again on the 23rd, payment due by the end of the month.
● Late Payment Stage 2: If previous month's fee is not confirmed, the amount will be carried over and re-requested on the 8th of the following month (due by the 22nd). If payment is not confirmed by that deadline, the line will be forcibly terminated the next day.
ABOUT CONTRACT TERMINATION
To terminate your contract, please contact the CareEarth Mobile Support Center. Changes to your contract or cancellation can also be completed through your personal My Page.
● Upon cancellation of the service, customers are required to return the SIM card to the address specified by our company. If the SIM card is not returned by the designated deadline or is returned in a damaged or deactivated state, a SIM card damage fee may be charged.
● The cost of returning the SIM card shall be borne by the customer.
● If the customer cancels the service by porting their number to another carrier via MNP (Mobile Number Portability), the service cancellation date will be the date the MNP procedure is completed. The usage fee for the month of cancellation will not be prorated.
ABOUT FILTERING SERVICE
In accordance with the “Act on Development of an Environment that Provides Safe and Secure Internet Use for Young People,” filtering services should be used as a general rule to prevent minors under the age of 18 from being exposed to harmful content, content that encourages illegal activity, or participation in unlawful actions.
CONTRACT DELIVERY VIA ELECTRONIC METHODS
● In accordance with the Telecommunications Business Act, contract documents issued after agreement will be provided electronically, rather than in paper form. The contract details will be made available in PDF format on the customer's My Page.
● If you wish to receive a paper copy of these documents, please contact our support center (Phone number: 0120-934-157).
ABOUT CHANGES TO TERMS OF SERVICE
If there are changes to the terms, conditions, policies, or other service rules, we will notify customers in advance by publishing the changes on our website, sending an SMS, or via other means deemed appropriate. Please note that revised terms will apply to services provided on or after the effective date of the changes.
ABOUT PROMOTIONAL CAMPAIGNS
For details on promotional campaign contents and important notes, please refer to the dedicated campaign webpage or flyer for each service.
INITIAL CONTRACT TERMINATION POLICY
※ This initial contract termination policy applies only to individual customers who have subscribed to an eligible “Voice Call Plan” (hereinafter referred to as an "Eligible Plan"). Customers with a plan other than an Eligible Plan are not covered under this policy.
The date the customer receives the “Detailed Contract Explanation” from us—after registering for the service—marks the beginning of the applicable period (for customers who opted to receive this document electronically during the sign-up process). Within eight (8) days from either the contract registration date or the service start date (whichever is later), customers may cancel their contract under this policy (“Initial Contract Termination Period”) by submitting a written notice of cancellation. The notice must include the registration date, registered name, address, phone number, and the name of the service to be cancelled. It must also be signed and sealed, then mailed to the designated address below. This cancellation is based on the Initial Contract Termination Policy as stipulated in the Telecommunications Business Act.
Furthermore, if the customer transferred from another carrier using MNP at the time of contract registration and wishes to cancel under this policy, the MNP transfer procedure must be completed during the Initial Contract Termination Period.
Please note that customers must complete the process themselves via their My Page to obtain an MNP Reservation Number (MNP Yoyaku Bangō)—we cannot apply for this on your behalf. If cancellation is performed alongside MNP, the cancellation date under this policy will be either the date the MNP transfer is completed or the expiration date of the MNP Reservation Number, whichever comes first.
Also note:
● MNP Reservation Numbers cannot be reissued or extended.
● If the customer obtained a new phone number when registering for the Voice Call Plan and wishes to cancel under this policy, MNP procedures will not be available.

We recommend using a registered mail service that provides delivery confirmation.
In cases where our company has provided incorrect or misleading information regarding the cancellation of this service, which causes the customer to believe the provided details were accurate, resulting in failure to cancel within the designated time frame, we will issue a formal written notice stating that the contract may still be canceled. From the date the customer receives this document, they will have eight (8) days to cancel the contract by sending a written cancellation notice by post, as described above. The cancellation of the contract shall be deemed effective upon the submission of the written cancellation notice, in accordance with the above procedure.
In the event that the contract for this service is canceled, and except as otherwise permitted by the Telecommunications Business Act, we will not claim compensation for damages or impose any penalties related to the cancellation. Additionally, if any payment has already been received by our company—beyond what is deemed payable—we will refund such amounts to the customer.
Even upon cancellation of this service contract, the customer shall remain responsible for the following fees:
● Monthly service charges (subject to change depending on the subscribed plan)
Please be advised that promotional campaigns offered by our company (including, but not limited to, waivers of service fees, registration fees, etc.) are not applicable to customers who cancel their contracts under the Initial Contract Termination Policy. Moreover, if the customer has used optional add-on services, fees will apply for each relevant function, including but not limited to call charges, SMS fees, SIM card replacement fees, SIM card switching fees, eSIM reissuance fees, and eSIM conversion/issuance fees.
Please note: Once the contract for this service is canceled, it cannot be reinstated in its original form. For additional important information regarding contract cancellation, please refer to the section titled “About Contract Termination” above.

Call and Communication Charges
※1 This section primarily describes call charges and communication fees. Please refer to the Terms and Conditions for detailed information. ※2 The international call rates above apply to daytime calls on weekdays to the United States (mainland) and vary depending on the destination country and time zone. For further details, please refer to the "Terms of Use for International Telephone Services."
※3 The communication fees stated above are limited to sending messages within Japan only. ※4 Charges vary depending on the number of characters sent. Additionally, the maximum number of characters that can be sent in a single message may differ depending on the device model or application used. ※5 Countries reachable via NURO mobile may differ from those available for regular international calls. For details, please check the pricing table in the Terms of Use. ※6 For customers subscribed to the fixed-rate calling option (Hōdai calls), the allotted seconds per option will not be added to the total usage seconds for billing calculation.
